Spirit of Women Awards Nomination Form

The Spirit of Women Awards recognize women who do extraordinary things for their communities, but who have received little or no public recognition for their contributions. Regional winners in three categories – Youth, Community and Healthcare Provider - will be honored at a Spirit of Women awards presentation in May. Each will receive a gift of $250 and will automatically become a nominee for a national Spirit of Women Award. Join Spirit of Women in celebrating women who dedicate themselves to making their communities a better place for all. Self-nominations will also be accepted.

Youth Recipient, 14-21: In terms of life experience, this young woman may be lacking in longevity, but she more than makes up for it with her intensity. Remember what it's like to be in the middle of, or at the end of, adolescence? Everything is a crisis. Everything requires your immediate attention and unwavering focus. But somehow this individual has the time and energy to extend her gifts to other people. She participates in, or maybe has even organized, a group that shares information, material goods, education, entertainment or inspiration with the people who need them. In her home, her school or her community, she's a good listener who knows how to turn dreams into reality.
Community Recipient, 22 and over: In this category, nominees understand clearly and passionately that how we perform as individuals determines how we perform as a community. She may not put that particular concept into words, but she certainly puts it into action. And action is what she's all about. She knows how to take the initiative, to pick up a phone, to pick up a pen, to pick herself up and do it. In fact, she searches for things that can't be done and does them. What distinguishes her from a lot of other people with convictions? What makes her different from the rest of us who will take action for the right cause? She's not afraid to fail.
Healthcare Provider: By the very nature of their work, healthcare providers are supposed to have attributes that set them apart from most other people. But even in this highly illuminated environment, there are some lights that shine brighter than others. The real test of this nominee's work can be measured by the care she gives to the job — or to the person — in front of her, even when myriad demands and commitments are swirling around her. She pays attention to detail, and in defiance of a popular self-help guide, she does sweat the small stuff. Whether it's within her clinical area or part of her life outside the organization, she would gladly sign her name to everything she does.
